Here is the pattern you will need. Your cardstock is cut to 5 1/2" x 3 1/4". So that you could see the score lines, I have made them solid for this photo!! Score your 5 1/2" side at 2 1/4", 2 3/4" and 5". Score your 3 1/4" side 1/2" in from both sides.
